Patient Forms
New Patient Forms are now available! You can click on the 'Patient Forms' tab to go to our secure patient registration portal. SKY Pediatric Dentistry has been designed to be as paper-less as possible. This reduces the amount of paperwork you need to fill out for your child at each visit and makes it easier to register your child or children for our practice. If you prefer, you can also fill out the forms, print them out and bring them in. Thank you and please contact our office if you have any questions at all.
Payment Options
For your convenience, we accept cash, personal checks, money orders, debit Visa cards and most major credit cards. Payment is expected at the time services are performed. Feel free to contact our office if your treatment plan requires more comprehensive dental work and you want to discuss financial arrangements.
